Make a Consolidation Request

The last date for Class Members to submit requests to consolidate their multiple claims forms was March 31, 2006. Class Members with multiple claim forms who have not submitted a consolidation request should file all of their claim forms.

Class Members who submitted a consolidation request by March 31st do not need to take any other action until they receive a consolidated claim form. These Class Members will have 90 days from the mailing of the consolidated form to submit their claim form or to challenge their estimated cash payment.
